پژوهشگران دانشگاه ایالتی کالیفرنیا و دانشگاه کالیفرنیا ایالات متحده امریکا در پژوهشی مشترک به بررسی انواع ولع به الکل در اعتیاد به الکل پرداختند.
روش:
در این پژوهش ۲۹۵ نفر دارای اعتیاد به الکل در بازه سنی ۲۱ تا ۶۵ سال که بیش از ۴۸ بار در ماه الکل مصرف نموده و طبق ملاکهای تشخیصی DSM اعتیاد به الکل داشتند شرکت نمودند. از میان آنها، ۴۳ نفر که تا بحال سابقه ترک نداشتند برای پژوهش انتخاب شدند.
ابزارهای پژوهش شامل پرسشنامه اطلاعات جمعیتشناختی، مصاحبه بالینی ساختاریافته بر اساس DSM-IV، مصاحبه بررسی خط سیر زمانی ۳۰ روز اخیر، مقیاس سنجش بالینی ترک الکل (CIWA-Ar)، پرسشنامه افسردگی بک (BDI-II)، پرسشنامه اضطراب بک (BAI)، مقیاس ولع به الکل پنسیلوانیا (PACS)، مقیاس نوشیدن وسواسی الکل (OCDS) و آزمون وابستگی به نیکوتین فاگر اشتغوم (FTND) بودند.
نتایج:
- ولع به الکل در شکل ناگهانی خود توسط مقیاس ولع به الکل پنسیلوانیا (PACS) بخوبی قابل تشخیص و پیشبینی است.
- ولع نسبت به الکل به شکل ناگهانی توسط رفتارهای وسواسی سوءمصرف قابل پیشبینی است.
- در ولع به الکل در شکل مزمن توسط مقیاس نوشیدن وسواسی الکل (OCDS) قابل سنجش و پیشبینی است.
- بطور معمول افرادی که نمرات بالا در ولع به الکل مزمن دارند، میزان بیشتری از ولع به الکل در اعتیاد به الکل به شکل تکانشی را از خود نشان میدهند.
راهبردهای کارکردی:
- یکی از علل افزایش سوء مصرف هر نوع مواد، بویژه سوء مصرف الکل و سوء مصرف نیکوتین/تنباکو افزایش استرسهای روانی-اجتماعی و ناتوانی افراد در مدیریت استرس است.
- آموزش مهارتهای دهگانه زندگی در سطح عمومی و به شکل مستمر منجر به تقویت سازوکارهای مقابله درست با مسایل و کاهش رفتارهای اعتیادی خواهد شد.
- آموزش مهارتهای دهگانه زندگی با کاهش سوء مصرف انواع مواد رابطه مستقیم دارند.
- مصرف الکل و اعتیاد به الکی در حقیقت نوعی رفتار وسواسی است که لازم است ریشههای اعتیادی آن در کنار روشهای گروههای ۱۲ گامی، در قالب محیطدرمانی و رواندرمانی فردی پیگیری شود.
Relationship between tonic and phasic craving for alcohol
Abstract
Background
Multiple measures are utilized to assess alcohol craving, often interchangeably. Little is known about the relationship between tonic and phasic craving. This study fills this gap in the literature by examining the association between tonic levels of alcohol craving and phasic craving for alcohol that is provoked by alcohol administration.
Methods
Forty-three non-treatment seeking problem drinkers underwent an initial interview and two laboratory testing sessions, where either alcohol or a saline placebo was administered intravenously.
Tonic craving was assessed via the Penn Alcohol Craving Scale (PACS) and Obsessive-Compulsive Drinking Scale (OCDS) at the initial interview.
Phasic craving was assessed during the laboratory sessions (i.e., alcohol and saline administrations, single blinded) at baseline and at 3 subsequent breath alcohol concentrations (0.02, 0.04, and 0.06 g/dl).
Results
There was a main effect of PACS in predicting phasic craving across both saline and alcohol administration conditions (p < ۰٫۰۵).
The OCDS was predictive of phasic craving when alcohol, but not saline, was administered (p = ۰٫۰۵۸); the obsessive subscale (p = ۰٫۰۱), but not the compulsive subscale (p > ۰٫۱۰), predicted phasic craving during alcohol, as compared to saline administration.
Conclusion
In sum, tonic craving captured by the OCDS was predictive of phasic craving during alcohol administration whereas the PACS more generally captured the increase in phasic craving.
Therefore, these measures of tonic craving may function differently in capturing the experience of phasic craving.
Implications for the utilization of the PACS and OCDS as well as assessments of craving in alcoholism research are discussed.
