پژوهشگران دانشگاه SEIN، و دانشگاه لایدن هلند، دانشگاه UCL انگلستان، دانشگاه RWTH آخن آلمان، دانشگاه ملبورن، و دانشگاه موناش استرالیا در یک پژوهش مروری مشترک به بررسی وضعیت شیوع، بروز، و روش‌های درمانی صرع در بزرگسالان پرداختند.

  • تعریف صرع در بزرگسالی شامل دو یا تعداد بیشتری تشنج بدون تحریک طی ۲۴ ساعت، یک تشنج بدون تحریک در صورت ریسک بالای تکرار (بیش از ۶۰% طی ۱۰ سال بعد)، یا تشخیص یک نشانگان صرع است. در صورتی که فرد بیش از ۱۰ سال بدون دارو هیچ تشنجی نداشته باشد، تشخیص صرع وی ملغی می‌شود
  • بیماری صرع در بزرگسالان یکی از رایج‌ترین وضعیت‌های حاد مغزی است که بیش از ۷۰ میلیون نفر در جهان درگیر آن هستند که ۸۰% آنها در کشورهای با درآمد پایین و متوسط هستند.
  • بیماری صرع بطور معمول از کودکی تا پایان عمر همراه فرد بوده و فرد مبتلا به آن در هر لحظه ممکن است دچار تشنج شود. این امر منجر به آسیب‌های بسیار زیستی-عصبی، شناختی، و روانی-اجتماعی برای فرد مبتلا و اطرافیانش می‌شود.
  • بیش از ۷۵% افراد مبتلا به صرع تا آخر عمر هیچگونه درمانی دریافت نمی‌کنند.
  • بررسی‌های ژنتیک نشانگر ساختار پیچیده انواع رایج صرع هستند. استعداد ژنتیک و وراثت نقش مهمی در صرع در بزرگسالی دارند.
  • بررسی‌های مستمر تصویربرداری مغزی، بویژه MRI و fMRI در کنار QEEG کمک شایانی به شناسایی بهتر علل کارکردی و پیش‌آگهی بیماری صرع در بزرگسالی و درمان می‌شوند. بررسی ابتلای همزمان به سایر مشکلات، بویژه ارزیابی‌های عصب‌روانشناختی منجر به سبب‌شناسی بهتر خواهند شد.
  • داروهای ضدصرع و ضدتشنج در دو سوم موارد منجر به پیش‌گیری از حمله‌های صرع در بزرگسالی می‌شوند، ولی پیش‌آگهی درازمدت بیماری را تغییری نمی‌دهند.
  • جراحی صرع موثرترین شیوه برای خلاص شدن طولانی مدت از تشنج است و در افرادی بکار می‌رود که صرع کانونی مقاوم به دارو دارند.
  • شناسایی عوامل محرک تشنج در هر فرد، یافتن زمان‌بندی رخداد تشنج فرد طی شبانه‌روز، و نیم‌رخ فعالیت هورمونی هر بیمار، می‌توانند به بهینه‌سازی زمان‌بندی مصرف داروها طی روز و در نتیجه حداکثرسازی پیشگیری از تشنج کمک شایانی نمایند.

Summary

Epilepsy is one of the most common serious brain conditions, affecting over 70 million people worldwide. Its incidence has a bimodal distribution with the highest risk in infants and older age groups.

Progress in genomic technology is exposing the complex genetic architecture of the common types of epilepsy, and is driving a paradigm shift.

Epilepsy is a symptom complex with multiple risk factors and a strong genetic predisposition rather than a condition with a single expression and cause. These advances have resulted in the new classification of epileptic seizures and epilepsies.

A detailed clinical history and a reliable eyewitness account of a seizure are the cornerstones of the diagnosis. Ancillary investigations can help to determine cause and prognosis.

Advances in brain imaging are helping to identify the structural and functional causes and consequences of the epilepsies. Comorbidities are increasingly recognised as important aetiological and prognostic markers.

Antiseizure medication might suppress seizures in up to two-thirds of all individuals but do not alter long-term prognosis. 

Epilepsy surgery is the most effective way to achieve long-term seizure freedom in selected individuals with drug-resistant focal epilepsy, but it is probably not used enough.

With improved understanding of the gradual development of epilepsy, epigenetic determinants, and pharmacogenomics comes the hope for better, disease-modifying, or even curative, pharmacological and non-pharmacological treatment strategies. Other developments are clinical implementation of seizure detection devices and new neuromodulation techniques, including responsive neural stimulation.
