آلکسی‌تیمیا و خودکشی نظامیان کره: اختلالات انطباقی و شخصیت

پژوهشگران دانشگاه سون چون هیانگ، دانشگاه نظامی دائه جون، دانشگاه کره، و دانشگاه کیونگ هی کره جنوبی در پژوهشی مشترک به بررسی وضعیت آلکسی‌تیمیا و خودکشی نظامیان کره جنوبی و ارتباط آن با اختلالات انطباقی و شخصیت پرداختند.

روش پژوهش:

در این پژوهش نظامیان مراجعه به بخش روانپزشکی بیمارستان ارتش کره جنوبی در بازه زمانی 12 ماهه مورد بررسی قرار گرفتند که در مجموع 215 نفر آنها واجد اختلالات انطباقی طبق ملاک های DSM بودند.

ابزارهای مورد استفاده شامل پرسشنامه اطلاعات دموگرافیک، مصاحبه مختصر روانپزشکی (MMSE)، مصاحبه مختصر نوروسایکیاتریک (MINI)، مقیاس افسردگی همیلتون (HRSD)، مقیاس سنجش آلکسی تیمیا، مصاحبه ساختاریافته DSM برای اختلالات شخصیت، مقیاس خلق و خوی کلونینگر (TCI) بودند.

یافته ها:

  • هر چه میزان نمرات آلکسی تیمیا در نظامیان کره ای بالاتر بوده، منجر به نمرات پایین در خرده مقیاس مشارکت و همکاری از پرسشنامه خلق و خوی بدست آوردند.
  • نمرات بالاتر آلکسی تیمیا در نظامیان کره ای رابطه مستقیم با شدت اختلالات انطباقی و سابقه اقدام به خودکشی در آنها دارد.
  • افزایش مشکلات در شناسایی احساسات، مشکلات در توصیف احساسات، و کاهش مشارکت و همکاری منجر به افزایش احتمال خودکشی می شود.

راهبردهای کارکردی:

  • اختلالات انطباقی و خودکشی در نظامیان به میزان زیادی تحت تاثیر مشکلات مدیریت هیجانی آنهاست.
  • مشکلات مدیریت هیجانی و تنظیم هیجانی به میزان زیادی ریشه در آلکسی تیمیا (بی واژگانی خلقی) دارد که به زبان ساده ضعف در هوش هیجانی است.
  • آموزش های بدو ورود و ضمن خدمت مدیریت هیجانی و تنظیم هیجانی به نیروهای مسلح منجر به مدیریت هیجانی بهینه آنها و در نتیجه افزایش سلامت و ثبات روانی و در عین حال افزایش روحیه تعامل و مشارکت آنها خواهد شد.

Alexithymia and low cooperativeness are associated with suicide attempts in male military personnel with adjustment disorder

Abstract

Subpopulations of patients with adjustment disorder are at increased risk for suicide. The current study investigated whether personality traits, including alexithymia, temperament, and character, are associated with an increased risk of suicide in individuals with adjustment disorder.

Method

Age- and sex-matched patients meeting the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria for adjustment disorder with (n=92) and without (n=92) a history of suicide attempts were recruited for the present study.

Ninety-two healthy individuals who did not meet diagnostic criteria for Axis I or II diagnoses were used as controls.

The Toronto Alexithymia Scale-20 (TAS-20) and the Temperament and Character Inventory (TCI) were used to assess personality traits.

Results

Significantly higher total and subscale scores on the TAS-20, including on the difficulty-identifying-feelings (DIF) and difficulty-describing-feelings (DDF) subscales, and lower scores on the TCI cooperativeness subscale were noted in adjustment-disorder patients with previous suicide attempts.

In the multivariate regression analysis, high DDF and DIF and low cooperativeness increased the risk of suicide attempts in adjustment-disorder patients.

A subsequent path analysis revealed that high DDF had a direct effect on suicide attempts, whereas high DIF had an indirect effect on suicide attempts via low cooperativeness.
