پژوهشگران دانشگاه هاروارد، دانشگاه کلمبیا، دانشگاه میشیگان، دانشگاه استونی بروک، و دانشگاه جنوب غربی تگزاس ایالات متحده امریکا در پژوهشی مشترک با استفاده از هوش مصنوعی و یادگیری ماشینی به شناسایی انواع زیرگونههای شناختی افسردگی و الگوهای پاسخ دهی آنها به دارودرمانی با سرترالین پرداختند.
روش پژوهش:
در این پژوهش آزمایشی مقایسه ای، 200 نفر که طبق DSM-5 دارای ملاک های افسردگی عمیق (MDD) بودند، در این پژوهش شرکت کردند. ابزارهای پژوهش شامل پرسشنامه اطلاعات دموگرافیک، پرسشنامه افسردگی همیلتون، مقیاس سنجش پاسخ به پاداش، مقیاس توجه دیداری-فضایی، پرسشنامه استدلال منطقی، پرسشنامه بازداری پاسخ، و آزمون روانی کلامی بودند.
یافته ها:
تحلیل هوش مصنوعی و یادگیری ماشینی نشانگر وجود 3 گروه متفاوت و متمایز در بیماران دارای افسردگی در پاسخ به سرترالین است.
یک گروه از افراد افسرده در توجه دیداری، روانی (سلاست) کلامی، و استدلال منطقی نسبت به باقی گروه ها تفاوت معنادار مثبت دارند.
آن دسته از افراد دارای افسردگی عمیق که بازداری پاسخ بهتر و پاسخگویی به پاداش ضعیف تر دارند، در مقایسه با دارونما، پاسخ بهتری به سرترالین می دهند.
راهبردهای کارکردی:
روش های یادگیری ماشینی و هوش مصنوعی، تکنیک های بسیار ارزان قیمت برای بررسی نتایج درمانی در اختلالات روانشناختی و روانپزشکی هستند که می توان به راحتی آن را در جمعیت های وسیع اعمال نمود.

Cognitive Subtypes of Depression are Associated With Response to Treatment by Sertraline
Background:
Major Depressive Disorder (MDD) involves neurocognitive impairments, but traditional diagnostic schemes fail to adequately capture these biological differences.
Here, we used unsupervised machine learning to classify depression based on distinct, biologically-meaningful behavioral targets and investigated the value of clusters in predicting antidepressant response.
Methods:
In a multisite, double-blinded clinical trial, MDD outpatients were randomized to 8-weeks of sertraline or placebo. Hierarchical linear modeling was used to analyze changes in Hamilton Depression Rating Scale.
At baseline, participants completed measures of reward responsiveness (RR), response inhibition (RI), visuospatial attention (VA), logical reasoning (LR) and verbal fluency (VF).
Gaussian Mixture Modeling was used to investigate potential cognitive clusters (after accounting for age, gender, education and site); and classification reliability was evaluated with rand, adjusted-rand and Jaccard’s indices via a jack knife procedure.
Finally, ANOVA examined whether improvements in depressive severity depended on treatment and cluster.
Results:
Three highly reliable clusters (N¼200) were found (rand¼0.97, adjusted-rand ¼0.94, Jaccard¼0.93). Cluster 2 (N¼90) had better RI and poorer RR than clusters 1 (N¼33) and3(N¼77), which did not differ. For VA, LR and VF, cluster 3performed the best, followed by cluster 2, and then cluster 1.
We found a significant Treatment Cluster interaction(F(2,189)¼3.17, p<0.05). Cluster 2 had greater improvement in depressive symptoms on sertraline compared to placebo but change in severity did not differ between treatments in other clusters.
Conclusions:
Classifying MDD based on behavioral measures, which are inexpensive and relatively simple to implement in clinics, might be useful for predicting antidepressant response.
